Required Clearances

Clearances must be within 12 months of issue and state they are for "employment" if applying for a position other than volunteer. The following clearances are required for all positions including substantial volunteers:

Required clearances

• Act 151 - Pennsylvania Child Abuse History Clearance (Form CY 113)

• Act 34 - Request for Criminal Record Check (Form SP-4 {164})

• Act 114 - FBI Fingerprint Clearance

Required forms/certificate of completion

• Act 24  - Arrest / Conviction Report and Certification Form (PDE - 6004)  Print and complete (all pages are required).  

• Act 168 of 2014 - Sexual Misconduct/Abuse Disclosure Release
  * Print and complete section one, then submit all pages to the district office.  We will forward the from(s) to our current/former employers.  

• Act 126 of 2013 - Mandated Child Abuse Recognition & Reporting Training certificate training at  reportabusepa.pitt.edu

• School Personnel Health Record Form/TB Test Results
