Mask Exemption Requests

Please be aware that evidence must be submitted along with your mask exemption form request.
Request for Mask Exemption Evidence
Dear Parent or Guardian:
We have received your child’s face mask/covering exemption request form, but we have not yet received any documentation supporting your request from a medical or mental health provider.
Dr. Sherri Smith, Acting Deputy Secretary for the Pennsylvania Department of Education, informed school districts on Friday, September 10, 2021 that we must require parents to submit “evidence that the student has a medical or mental health condition or disability that precludes the wearing of a face covering.”
Please submit documentation as evidence supporting your request for your child to be exempt from the masking mandate.
We are now required by the Commonwealth of Pennsylvania to have documentation supporting your request for your child to be exempt from the masking mandate on file. Please submit your evidence, such as a note from your child’s medical or mental health provider to your child’s principal by September 17, 2021.
Please be aware that in the absence of such documentation, we will have to require that your child wear a mask in school. Thank you for your prompt attention to this matter.
